Community Health Assessment
The community health assessment (CHA) is required to be conducted every 5 years at the health department level; non-profit hospitals are required by the IRS code to complete a community health needs assessment (CHNA) every 3 years. Often times, these assessments are completed in partnership and therefore our CHA for the Pittsylvania-Danville Health District is usually completed every 3 years.
The City of Danville Community Health Assessment Report was published by the Pittsylvania-Danville Health District with data from the City of Danville Assessment in 2024.
The Pittsylvania County Community Health Assessment was pulled into the 2024 Centra Lynchburg Area Community Health Needs Assessment (CHNA) Report.
The Community Health Improvement Plan (CHIP) is the follow-up actions on the community member identified priorities led by work with community partners and volunteers. We are currently searching for organizations and individuals who would like to assist with this project. Pittsylvania County’s current priority areas are Food Insecurity and Nutrition; Homelessness & Housing; and Access to Healthcare Services. Danville’s current priority needs are Access to Healthcare Services; Aging & Eldercare; and Chronic Disease.
