“Wait – Do I Need PPE for This?” Making Sense of Enhanced Barrier Precautions

Infection prevention in long-term care takes teamwork—and sometimes a little extra reinforcement. It’s been a year since CMS formally added Enhanced Barrier Precautions (EBP) to infection control guidance, and that big question— “Do I need PPE for this?”—still comes up.

EBP expands personal protective equipment (PPE) use beyond isolation, applying to high-contact care like wound care, changing linens, and bathing. EBP is key to stopping the spread of multidrug-resistant organisms (MDROs) in long-term care facilities, but it takes clear systems and consistent staff education to make it stick.

What to Expect: This month’s resources are designed to meet your team where they are—whether you’re providing direct care, coordinating therapy, or supporting infection prevention efforts facility-wide.

How to Use These Resources:

  • Share the 8 Moments visual in breakrooms, care stations, huddles, or staff emails.
  • Use the algorithm and observation tool to support your facility’s infection control plan or prep for survey readiness.

Enhanced Barrier Precautions are a big shift, but they’re not a solo effort. Whether you’re providing direct care or leading infection prevention and control (IPC) efforts, these tools are here to help you do the work and explain the why. Infection prevention is always evolving, but so are we.

Toolkit for Improving Skin Care and Multidrug-Resistant Organism (MDRO) Prevention in Long-Term Care Settings   Clinical topics in the toolkit center around four key strategies to protect skin and prevent infection: (1) Keep skin clean and safe; (2) Reduce MDRO transmission; (3) Use antibiotics wisely; and (4) Clean high-touch surfaces.   The toolkit also includes “teachable moments.” These documents use real-world healthcare scenarios to reinforce concepts related to skin care and infection prevention.

This guidance document updates the 2008 SHEA/Association for Professionals in Infection Control and Epidemiology (APIC) guideline: Infection Prevention and Control in the Long-Term Care Facility and is intended to assist nursing homes in the United States in defining and implementing their infection prevention and control (IPC) programs and practices. Recommendations and implementation suggestions address IPC leadership, staffing, and resources, healthcare personnel and residents‘ adherence to precautions and effective hand hygiene, outbreak preparedness, training, occupational health, cleaning and disinfection in the care environment, and the involvement of IPC in the facility.

This clinician letter outlines updates to immunization schedules for MMRV (measles, mumps, rubella, and varicella) and COVID-19 vaccines, emphasizing their essential role in community health and safety in Virginia. It discusses the new guidelines, including the separate administration of the MMR and varicella vaccines for children under 4 years, the COVID-19 vaccine's availability for individuals aged 6 months and older, and the importance of verifying insurance coverage prior to vaccination.

VDH Infection Prevention Education in the Northwest Region

Kayleigh Rehkopf, VDH’s Northwest Regional Infection Preventionist, would like to highlight three VDH staff in the Northwest region (Stephanie Neal, Candace Ashworth, and Brad Evert) for their outstanding work in infection prevention and control (IPC) education!

Stephanie is the Junior District Epidemiologist for Lord Fairfax Health District, and she has been working on preparing and disseminating IPC education for nursing homes, assisted living facilities, and schools. Brad and Candace, part of the Regional Containment Team, eagerly jumped in to help with the education and went outside their normal scope of work to be a part of this important initiative.  They did a great job familiarizing themselves with the education and making connections in the community to deliver the content effectively. Having them on the team really helped to make this education effort a success.
