Visit to sign up for a COVID-19 vaccine, including booster doses and childhood vaccines for those ages 5+.

Update Regarding Booster Doses:

According to the latest CDC guidance, everyone 18 years and older is now eligible for a COVID-19 booster dose. 

These people should get a booster:

  • Anyone 18 and older who got the Johnson & Johnson vaccine;
  • Anyone age 50 and older who got the Moderna or Pfizer vaccine;
  • All residents 18 and older who live in long-term care settings

These people may get a booster:

  • All adults ages 18 and older at least six months after completing their primary series of Pfizer or Moderna

The FDA and CDC also authorized mixing and matching different vaccines from the initial series.

COVID-19 Community Clinics

CSHD will be offering mobile COVID-19 vaccine clinics in the community in an effort to increase access to COVID-19 vaccines. To succeed in these efforts, we need your help! If your organization is interested in a COVID-19 vaccine clinic for your community, then we are asking that you complete this request formYou can also go to mobile clinics can be walk-up with no registration needed and can be flexible in date and time of clinics. Our clinics can serve anywhere from 20-200 people. This service is free and available to all community members.

For some inspiration, a mobile clinic could be held at:

  • a local festival, event, block party, farmers market, etc.
  • a church, community center, business
  • a food pantry, homeless shelters
  • a park or public space
