Notice of Fee Adjustment
Effective July 1, 2026, the Virginia Department of Health and Department of Motor Vehicles will implement a fee rate adjustment increasing the Vital Record Certificate Fee to $15.00. In accordance with Chapter 616 of the 2026 Acts of Assembly, the $15.00 fee rate will apply to each certified copy of a vital record that is issued, or for each search of the system of vital records where a certified copy of a vital record is not made.

We are open for walk-in customers from Monday - Friday 8:00 a.m. to 4:30 p.m.

Customers may obtain an application for a vital record at the front desk. Acceptable applications must have all required sections filled out including:
- Requester’s signature.
- Legible photocopy of the requester’s ID.
Online Requests
Customers may also use the Virginia Department of Health's Office of Vital Records new fully online system that will allow you to apply for a vital record, pay for it, and receive updates.
Costs for Vital Records
The Code of Virginia requires a cost of $15.00 per certificate/search. There is no refund if the certificate is not found.
An administrative fee of $10.00 is charged for any change to a certificate.
Method of Payment: Check, money order, payment card, mobile pay, or cash. Checks and money orders should be completed, signed, and made payable to Prince William Health District.
Who Can Make a Request
Vital records are available to you, and immediate family members only: mother, father, husband, wife, adult son or daughter, brother, sister, grandparents and adult grandchildren with valid identification.
- Aunts, uncles, cousins, in-laws, ex-spouses, etc. cannot obtain a vital record. A non- family member is entitled to obtain a vital record only if they have notarized Power of attorney.
- Birth records become public information 100 years after the date of the event while death, marriage and divorce records 25 years after the date of the event.

Birth Certificates
Certified Copies of Birth Certificates
The PWHD Office of Vital Records has records of births that occurred in Virginia from June 14, 1912, to the present.
Records prior to 1912 must be requested through Virginia’s Division of Vital Records in Richmond
Please visit our office during normal business hours to obtain a certified copy of a birth certificate.
Important: Please note birth cards are no longer issued. The PWHD Office of Vital Records does not offer apostilles or seals on a birth certificate or other vital record. More information about how to obtain this can be found at the Office of the Secretary of the Commonwealth.
Correcting a Virginia Birth Certificate
To amend a birth certificate, parties are encouraged to write to Vital Records with their correction request before going to the Circuit Court.

You may request certain corrections to a birth certificate through Virginia’s Division of Vital Records. If the request was denied by the Division of Vital Records, you may file a request through the Prince William Circuit Court or with the Circuit Court in the jurisdiction where you reside. A copy of the denial letter from the Division of Vital Records must accompany the petition, order and any supporting documentation to correct a birth certificate.

Correction of a Virginia Birth Certificate
Death Certificates
What the PWHD Office of Vital Records can do:
- Provide copies of death certificates for all localities in Virginia from June 14, 1912, to present.
- Certify death certificate applications.
- Issue a Verification of Death Letter.
- Issue out-of-state transit permits.
- Application for Vital Records are $15.00 per copy.
Marriage Certificates
The PWHD Office of Vital Records can provide certified copies of marriage certificates that occurred in Virginia from 1853 to the present.
The following information is required to complete the application:
- Full name of the groom
- Full maiden name of the bride
- Exact date (month, day, and year) of marriage
- Place of marriage
- Name of the circuit court that issued the marriage license.
The Virginia Department of Health's Office of Vital Records handles the filing of corrections to marriage records. In order to correct these records, you should first contact the city or county circuit court that issued the marriage certificate.
A Single Status Letter is to verify that no current marriage is on file in the Commonwealth of Virginia. This document is usually needed for marriages that will occur abroad. In Party One of the Marriage Information section of the application, list the name of the person requiring the single status letter. Attach to the application a signed letter with the required information. To obtain a Single Status Letter, contact the Virginia Department of Health's Office of Vital Records located in Richmond by calling 804-662-6200.
Divorce Certificates
The PWHD Office of Vital Records can provide certified copies of divorce certificates that occurred in Virginia from 1918 to the present.
The following information is required to complete the application:
- Full name of the groom
- Full maiden name of the bride
- Exact date (month, day, and year) of divorce
- Place of divorce
- Name of the circuit court that filled the divorce.
If you need a Divorce Decree, please contact the courthouse where your divorce is filed.
