This feature provides you with the ability to retrieve a PDF copy of your online application. You will be asked for the Application Tracking ID issued by the Office of Vital Records, Requester Email address provided in the application, and the Registrant Last Name. You can find the Application Tracking ID in the email notification received upon successful registration of your application.
If you previously selected to make your payment online but for any reason were not able to complete the transaction, when you retrieve your application you will have the option to complete the online payment or to switch to 'offline' payment (that is, submit your payment along with the signed application rather than continue with the online payment).
