Perinatal Health Hubs Pilot Program

The Perinatal Health Hubs Pilot Program will provide $2.5 million in grants to organizations for a two-year period to expand perinatal health hubs across the state. Perinatal health hubs provide community-based, wraparound supports and services to pregnant and postpartum women and their infants. For the purpose of this funding opportunity, a perinatal health hub is defined as:

  • Community-based, located in the area it serves
  • Caring for the mother-baby dyad during pregnancy and through at least 1 year postpartum
  • Providing multiple services for families (see online application for specific services)
  • Focused on reducing maternal and infant morbidities and mortalities

The ultimate goal of this work is to help reduce maternal and infant mortality and morbidity in the Commonwealth.

This program is made possible by Chapter 725 of the 2025 Virginia Acts of Assembly (Budget Bill, Item #277H).

Rockbridge Area Health Center

The Perinatal Health Hub at Rockbridge Area Health Center coordinates a multi-disciplinary team to provide compassionate, family-centered care to support mothers and babies through every stage of pregnancy and early childhood. Our services include prenatal education, obstetric care, lactation support, postpartum care, well-child visits, and behavioral health therapy designed to promote healthy families and strong beginnings. Through personalized support, education, and integrated healthcare services, we are committed to improving access to quality perinatal care for families across our community.

Child Development Resources

Child Development Resources (CDR) Motherhood Program, in partnership with Sentara Williamsburg Regional Medical Center will ensure that every mother who gives birth in our community will receive information about available services and an invitation to an initial home visit (virtually or in person) for a discussion of resources needed and referral to appropriate programs. As part of the VDH-Perinatal Health Hub pilot, CDR is expanding outreach and services to serve a broader region and to offer new services to teen mothers, Spanish-speaking families and others.

Johnston Memorial Hospital

The VA Perinatal Hub at Johnston Memorial Hospital will be offering additional services through their already existing Strong Starts program. Additional services include Maternal Certified Peer Recovery Specialists (CPRS), Birth Doula support, and enhanced childbirth and parenting education for patients and community members.

Strength in Peers

Strength in Peers

The Rocktown Maternal Health Hub is a partnership between Strength In Peers and Sentara RMH Medical Center to addresses the high rates of late prenatal care, substance use, poverty, and language barriers that put mothers and infants in the Shenandoah Valley region at risk. Through community outreach, a walk-in resource center, care coordination, peer recovery support, health education, and doula care, the Hub connects women to the services from a first pregnancy test through a year postpartum, with the goal of reducing preventable maternal and infant complications.
