BRHD Community Health Workers (CHW) Program

Community Health Workers (CHWs) are an integral part of Public Health 3.0 (Public Health 3.0: A Call to Action for Public Health to Meet the Challenges of the 21st Century), where cross-sector collaboration is utilized to combat inequity and build healthy communities. By identifying barriers to care and bridging the gap between services locally, BRHD’s CHW team is putting the pedal to the metal for a new & improved public health system!

We have 4 CHWs, 3 are bilingual Spanish speakers.  Check back on this page to find out more about our CHW team and upcoming events.

The Community Health Workers will be at each Health Department site alongside BRHD’s Child & Maternal Health program, providing education on Safe Sleep and FREE portable cribs. Access the flyer HERE to register for the event and reserve your crib. 

CHW Office Hours

BRHD CHWs are hosting Office Hours routinely across the District to connect/refer community members with services and resources (like housing or food), provide education on different health topics, translation services, and a range of other types of support and assistance.
