Anyone making an appointment for pneumonia vaccine, either PCV13 (pnemococcal conjugate vaccine) or PPV23 (pnemococcal polysaccharide vaccine) must bring a prescription from their clinician indicting which pneumonia vacci
ne they would like their client to receive.

Infants, children, adolescents and adults. Please bring a copy of your immunization record if available.
Fees
Required childhood immunizations are provided at no charge. All other vaccines have a charge.
