Renewing Your Foodservice Facility Annual Permit

Prior to the expiration date noted on your VDH Permit to Operate a Foodservice Establishment you will need to renew your permit every year. At the end of each quarter, the Blue Ridge Health District will mail or email an invoice to permit holders, advising them to renew their existing permits. To avoid operating without a valid permit, the annual permit must be renewed prior to the expiration date.

Follow these steps to submit your renewal application:

STEP 1:  Complete the Annual Permit Application

You will need to complete the Annual Food Service Permit Application and pay the $40 annual renewal fee.

Be sure to include current mailing address so you can keep up with new information and for billing purposes.

STEP 2:  Submit your completed Annual Permit Application

  • Scan it into your computer and email to your local health department where the facility is located or based Local health department contact information You can then follow up by calling the locality where your operation is based to pay the $40 annual permit fee with a credit card; or
  • You may also mail the permit to your locality office along with a check; or
  • Submit the application and pay the fee directly at the locality office.
