WHAT IS COMMUNITY HEALTH IMPROVEMENT?

Our Community Health Improvement Team (formerly referred to as "population health") works to prevent diseases, promote healthy lifestyles, and address health inequities in the community where we live, learn, work, and play.  The team focuses on understanding the conditions and factors that influence the health of specific communities and populations around the Central Virginia Health District.

COMMUNITY HEALTH ASSESSMENT

The CVHD population health team is part of the Partnership for Healthy Communities, a group of local stakeholders who lead the community health assessment and improvement plan process. This collaborative process resulted in the 2021-2024 Community Health Assessments found here:
