The CHA in Action!

The Community Healthcare Coalition of Greater Prince William (coalition) alongside the Prince William Health District have been on a mission to hear from as many voices as possible in Greater Prince William through their CHA survey and community conversation efforts.

Thanks to the incredible support of community members, leaders, agency and community-based organization partners we've reached people in multiple ways: participating in outreach events, conducting listening sessions known as community conversations, and administering a community health survey in person and online.

Below is a look at everything we did to make sure YOUR health and voices were heard!

Core Tenet Guiding the CHA

Health accountability is the shared responsibility of healthcare systems, public agencies, and community stakeholders to ensure transparent, inclusive, and sustained actions that promote attainment of the highest level of health for residents in Greater Prince William.

The 2025 Greater Prince William Community Health Assessment (CHA) reveals that residents call for coordinated action to address persistent barriers to health and well-being. Lasting health improvements require more than reporting problems -- they require multi-sector partnerships.

The Coalition is sharing data findings and mobilizing across sectors this fall in preparation for a January 2026 meeting. At this meeting community partners and residents will choose health issues to prioritize for the Community Health Improvement Plan (CHIP). We want to work with a variety of community partners and residents to ensure additional perspectives are incorporated into the CHIP to ensure it reflects the diversity of our population.
