The Community Healthcare Coalition of Greater Prince William (coalition) alongside the Prince William Health District have been on a mission to hear from as many voices as possible in Greater Prince William through their CHA survey and community conversation efforts.
Thanks to the incredible support of community members, leaders, agency and community-based organization partners we've reached people in multiple ways: participating in outreach events, conducting listening sessions known as community conversations, and administering a community health survey in person and online.
Below is a look at everything we did to make sure YOUR health and voices were heard!
Core Tenet Guiding the CHA
Health accountability is the shared responsibility of healthcare systems, public agencies, and community stakeholders to ensure transparent, inclusive, and sustained actions that promote attainment of the highest level of health for residents in Greater Prince William.

Next Steps
The 2025 Greater Prince William Community Health Assessment (CHA) reveals that residents call for coordinated action to address persistent barriers to health and well-being. Lasting health improvements require more than reporting problems -- they require multi-sector partnerships.
The Coalition is sharing data findings and mobilizing across sectors this fall in preparation for a January 2026 meeting. At this meeting community partners and residents will choose health issues to prioritize for the Community Health Improvement Plan (CHIP). We want to work with a variety of community partners and residents to ensure additional perspectives are incorporated into the CHIP to ensure it reflects the diversity of our population.

Frequent outreach took place between December 2024 and February 2025 at local WIC offices, federally qualified health centers, and community non-profit organizations:

We wanted to do more than just collect survey responses-we wanted real conversations. Through 17 listening sessions, we are hearing firsthand about experiences, and concerns people face when it comes to achieving community health and ideas on how to improve programs and services. These conversations give us a deeper understanding and context behind the survey responses and were hosted by the following partner organizations:

PWHD's Community Engagement team canvassed over 70 businesses and community spaces in Greater Prince William. Thank you to partnering organizations who led their own CHA neighborhood/off-site canvassing efforts to reach non-English speaking residents:

We used every tool available at our disposal to engage community members who live in Greater Prince William, including organic social media promotion, email outreach, and website spotlights. Through these efforts:
- Total of 13 social media posts shared by PWHD on a bi-weekly basis on Facebook, X, and Nextdoor.
- Combined total of over 62,000 impressions (views)
- Spotlighted 3 times on PWHD's newsletter
- Community partners amplified the message by re-sharing or uploading posts and newsletters.
