The Diamond Hil neighborhood was chosen based on data showing that over 53% of adults 18 and older have high blood pressure in that census tract.

Healthy Heart Ambassador Blood Pressure Self-Monitoring Program

This is a four-month, evidence-based program focusing on home blood pressure monitoring and nutrition education. Our chronic disease prevention community health worker is certified to facilitate this program and to train other facilitators.

 

Quit Now Virginia

This state program provides free information and coaching by telephone, text or online to residents who want to quit using tobacco or nicotine products. The counseling is offered by trained Quit Coaches; in combination with nicotine replacement therapy or medication prescribed by healthcare providers when needed. 1.800.QUIT.NOW (1.800.784.8669)

Diabetes Prevention Program

Trained lifestyle coaches facilitate the CDC Prevent T2 Diabetes Prevention Program at chosen locations in the district. Location examples include senior living facilities, churches, community centers, etc. Our program targets Medicare eligible community members who are pre-diabetic.
