Disease Reporting

Please use the form below to submit reportable diseases or conditions to the Virginia Department of Health.

If you are reporting a rapidly reportable condition, please call our District Epidemiologist, Cali Anderson, directly at (434) 363-0358.

Please enter as much information as is available in order to ensure that the health department is able to respond to your report effectively.

Unusual Occurrence of Disease of Public Health Concern

  • Immediately report unusual or emerging conditions of public health concern.
  • Unusual or emerging conditions of public health concern shall be reported to our Epidemiologist immediately by the most rapid means available, preferably by telephone.

In addition, the Commissioner or the Commissioner’s designee may establish surveillance systems for diseases or conditions that are not on the list of reportable diseases. Such surveillance may be established to identify and count cases, identify the mode(s) of transmission and risk factors for the disease, and implement appropriate actions to protect public health.

Outbreak Reporting

Certain facilities or programs, such as residential or day programs, services or facilities licensed or operated by any agency of the Commonwealth, schools, child-care centers, and summer camps are required to report the presence or suspected presence of an outbreak to the local health department per 12VAC5-90-90D . Other entities/businesses may voluntarily report outbreaks.

Foodborne Illness Reporting

If you think something you ate is making you or someone you know feel sick, contact your doctor and then fill out this My Meal Detective form as completely as possible. The Virginia Department of Health will evaluate your complaint and request follow-up by your local health department, if necessary. Your local health department may need to contact you for more information.
