Stroke Smart Proclamation Process 7-21-25: Start here: Explains how to get a proclamation

Proclamation Template: This template provides the suggested language for your city/county proclamation

Senior Health Leadership Email: A draft email to get a meeting with the senior leadership of your hospital or health system (i.e. CNO, CMO, CEO or President)

A Stroke Smart Community Action Group ("group") brings leaders and interested community members together to promote the Stroke Smart message.

Three key organizations to contact for members are:

• Local Government or Public Organizations - examples are Government official/office, libraries, public schools, public transportation, and recreation centers.
• Health Care Providers - examples are hospital system, primary care practices, stroke support groups, emergency medical services (EMS), and assisted living facilities.
• Community Organizations - examples are senior living complexes/apartments, civic associations, businesses, and faith-based organizations.

This letter can be modified to invite members to the group.
