Eligibility Information
Financial Eligibility is determined at each certification visit and whenever there is a change in household income. Proof of income is required at each certification visit. If you have questions about what to bring for your family please call your local health department WIC program.
Examples of Proof of Income
- Pay Stubs- provide either the most recent three months or only the most recent check stub if “year to date” income is listed on the check stub
- W-2 Form or income tax return for the most current calendar year
- Employer’s written statement regarding weekly, monthly or yearly income
- Statement of Social Security benefits
- Military LES (leave and earnings statement)
- Foster care statement from Social Services
- Food Stamps- Letter of eligibility
- AFDC check stub
- Medicaid card for the current month
- Free or reduced school lunch-letter of eligibility
- Minors if not working but provide parent’s income to determine eligibility
