Vital Records

Birth Certificate Applications and Death Certificates

Death records are filed at the health department in the country or city in which the death occurred, not the country or city in which the person lived. Certified copies can be obtained only by a relative of the deceased or by a representative of a funeral home. The Peninsula Health District has death records on file from 1980 to the present for deaths occurring in Newport News, Williamsburg, Poquoson, James City County and York County. The cost per copy of a death record is $12.00.

For deaths occurring in Newport News, Poquoson or York County, call (757)594-7314.

For deaths occurring in Williamsburg or James City County, call (757)253-4740.

Copies of DEATH records prior to 1980 and all BIRTH records are filed in Richmond and can be obtained by sending the completed request form to the following address:
