Applications for a Vital Record

Applications for a Vital Record can be obtained at any of our local health departments.  Vital Records are filed in Richmond and can be obtained by visiting the Office of Vital Records or by sending the completed request form to the address below. 

Death Certificates

Death records are filed at the health department in the county or city in which the death occurred, not the county or city in which the person lived.  Certified copies can be obtained only by a relative of the deceased or by a representative of a funeral home.  The cost for a death record is $12.00 per copy.  The Three Rivers Health District has death records on file from 2008 to the present for deaths occurring in Essex, Gloucester, King & Queen, King William, Lancaster, Mathews, Middlesex, Northumberland, Richmond and Westmoreland counties.  Copies of death records prior to 2008 are filed in Richmond and can be obtained by visiting the Office of Vital Records or by sending the completed request form to:
