What is WIC?

WIC is a Special Supplemental Food Program for Women, Infants, and Children.

Who is eligible for WIC?

Women:

Pregnant, Post Partum or Breast-feeding

Infants:

Under one year of age

Children:

Under five years of age – once turns five years old, he/she is no longer eligible for the WIC Program.

All applicants must qualify both financially and nutritionally.

What you need to bring to your appointment

• Infants must be physically present at certification
• Your child (over the age of one) or a medical form that is no older than 60 days, from a doctor which includes a height, weight and hemoglobin/hematocrit.
• Proof of income for entire household
• Food Stamp Notice
• TANF
• Medicaid Card
• School Lunch Notice
• CMSIP
• Current Paystub/W-2
• Proof of identification
• Social Security Card
• Medicaid Card
• Driver’s License
• Birth Certificate
• Crib Card
• Proof of residency (911 & Mailing Address)
• Driver’s License
• Rent/Mortgage Receipt
• Medicaid Card
• Utility Bill
• WIC folder (Recertification Only)
• Immunization record

• Call your local health department between 8 a.m. – 4:30 p.m.
