COVID-19 SPECIAL STATEMENT:
Our Vital Records office is closed to in-person visits. However, we are accepting requests by mail and fax (804) 786-9007.
The Vital Records Office records deaths and issues death, birth, marriage and divorce certificates. The cost for certificates is $12 per copy.
Birth, Marriage, and Divorce certificates are also available from the Commonwealth of Virginia Department of Health.

How to Apply in Person for a Vital Record

In-person requests are currently not being accepted due to COVID-19. FAX the completed form along with a copy of an acceptable form of personal ID to our office fax number: (804) 786-9007, or mail to our Cary Street address.

You can now create the application form for a vital record online. We recommend using this NEW option for faster processing of your application. Once you generate the Application online, the Office of Vital Records issues an Application (Tracking) ID to track your application status online. You will receive email/SMS updates on the status of your application by providing your email address and mobile phone number.
The Code of Virginia requires a cost of $12.00 per certificate/search. There is no refund if the certificate is not found. A fee of $10 is charged for making the amendment to an existing record.
