General Grant Information

ALL GRANT APPLICATIONS MUST BE SUBMITTED ELECTRONICALLY UNDER THE GENERAL FUND, HOWEVER THE ORIGINAL AFFIRMATION PAGE MUST BE RECEIVED BY MARCH 16, 2009 WITH THE ORIGINAL SIGNATURES OF THE (1) AUTHORIZED AGENT, (2) FISCAL OFFICER AND (3) OPERATIONAL MEDICAL DIRECTOR. IF NOT RECEIVED BY MARCH 16, 2009 YOUR GRANT WILL BE DISQUALIFIED! FAXED AND EMAILED AFFIRMATIONS WILL NOT BE ACCEPTED!

NOTE: ALL SUPPORTING DOCUMENTATION, IF APPLICABLE MAY BE EMAILED, FAXED OR MAILED:

  • IF A APPLYING UNDER ONE OF THE SIX PRIORITY LEVELS – SPECIAL PRIORITIES QUESTIONNAIRE
  • IF APPLYING FOR RESCUE TRUCK/EXTRICATION EQUIPMENT – RESCUE/TRUCK EXTRICATION EQUIPMENT QUESTIONNAIRE
  • ANY ADDITIONAL NARRATIVE, PROJECT DESCRIPTION, SUPPORT LETTERS OR QUOTE

State Contract Purchases

Effective January 1, 2009 quotes will be required for all items not listed on the Commonwealth of Virginia procurement website for a state contract www.eva.virginia.gov. Currently, Quotes are HIGHLY RECOMMENDED for all grant items requested! Currently the Commonwealth of Virginia does not have a State Ambulance Contract, however pricing is based on the OEMS price list for vehicles and equipment.

Priority Funding

The OEMS has identified six priority levels for funding in no particular order:

  • Recruitment and Retention (Including Management, Leadership and Training)
  • Emergency Operations
  • Emergency Medical Dispatch
  • Special and Innovative Projects
  • Multi-Jurisdictional (Multi-Agency) Projects

The OEMS will determine if your grant application falls within one of the priority funding categories. All grantees that are applying under the intent of a funding priority category must submit the Special Priorities Questionnaire. You can download the Special Priorities Questionnaire from the downloads section below.

2008 Reimbursement Information

Effective December 1, 2007, in order to be reimbursed you must submit the 2006 IRS Form 990 copy (1st page only) of your tax returns. All information must be filled out including items (C) (D) and (E), without these items completed you WILL NOT receive reimbursement for your grant.
